To ensure successful completion of your case, please complete the following steps:

1. Carefully fill out your Rx, being sure to include Dr. Name, Patient Name, Shade, Tooth # and Type of Restoration Material.

2. Carefully package your case, including the Rx, and tape box securely closed. Next, place casebox into FedEx envelope. Seal envelope and apply FedEx airbill. Keep your tracking number.

3. Call Customer Service at 1-800-479-5203 to schedule your case pick-up.

4. Track the shipping status of your account at www.fedex.com or by calling Smith-Sterling Customer Service at 1-800-479-5203.

5. Your case will be returned via overnight courier service in just 9 working days. Working days do not include weekends or holidays.
